RoundingWell Customizes Platform in Record Time to Support the Vanderbilt Health Affiliated Network’s Care Management Programs

NASHVILLE, Tenn. April 6, 2021 – RoundingWell, a Nashville-based digital health company, recently completed a first-of-its-kind customization of its platform to support the Vanderbilt Health Affiliated Network (VHAN)’s industry-leading care management team. Teams from both companies collaborated to adapt RoundingWell’s platform into a highly personalized, integrated tool that could communicate and coordinate across the multidisciplinary interventions VHAN delivers to its patients in partnership with their local healthcare providers. 

The VHAN care management team, which includes pharmacists, social workers, nurses and other clinical experts, provides enhanced support for patients with complex and/or chronic medical needs. The collaboration with RoundingWell has resulted in a tailor-made tool VHAN uses to build a hyper-targeted outreach strategy and more effectively coordinate efforts among the various interventions. With its data all in one place, VHAN can now track and measure health improvements from its coordinated interventions and see a more accurate snapshot of each patient. These valuable data points can also be shared with health plan leaders and insurance carriers to ensure every patient is getting the support they need to achieve their health goals.

This is only the beginning of the collaboration between the two Nashville-based entities. In addition to expanding its clinical programs, VHAN will be looking to RoundingWell for patient outreach assistance for several health prevention programs in development.
